Privacy Notice

We want you to be absolutely confident that we are treating your personal data responsibly and that we are doing everything we can to make sure that only the people who can access that data have a genuine need to do so.

 

Yasmin Hodge is the Data Controller and therefore decides how your personal data is processed and for what purpose.

What is personal data and why do we collect this information?

Personal data relates to a living individual who can be identified from that data.  Special category data is a sub-category of personal data and includes data relating to health.

What data do we collect and why?

To provide the best care, personal information about your health is collected. By agreeing to a treatment and a treatment being provided, a contract is formed. You can refuse to provide the information, but if you were to do so we would not be able to provide treatment.

We have a legitimate interest in collecting that information because without it we couldn’t do our job effectively and safely.

We think it is important that we can contact you to confirm your appointments or to update you on matters relating to your care. This is in your legitimate interest.

Provided we have your consent, we may occasionally send you clinic updates. You can withdraw this consent at any time, just let us know by email: yasminhodgeacupuncture@gmail.com

How long do we keep your personal information?

We are legally required to keep client records for a minimum of six years after your most recent appointment. Under British Acupuncture Council regulations, notes are kept for seven years. 

 

In the case of minors, records must be kept until the patient reaches the age of twenty-five (seven years after reaching eighteen). 

 

After this period, your notes, email communications and text messages will be destroyed. New notes will be set up if you do wish to return for appointments at the clinic.

Sharing your information 

Your data is strictly confidential.  We will never share your data with third parties without your consent. Only Yasmin Hodge has routine access to your data to provide your treatment.

If you are claiming back payment of the fee from an insurance provider, that insurance provider can request confirmation of payment for treatment, the dates of treatment and the amount paid.  Insurance companies will have included the right to confirm treatment and payment within their terms and conditions. The practice will not disclose any personal details but will confirm payment and dates of treatment should this information be requested by the insurance provider.

In the event of an emergency, your name, address, DOB and emergency contact will be provided to emergency services. Your emergency contact will also be informed.

Your rights

You have the right to see what personal data of yours we hold.  You can ask us to correct any factual errors. Provided the legal minimum period has elapsed, you can ask us to delete your records.

Complaints

If you feel we have mishandled your personal data in some way, you have the right to complain.  Complaints should be sent to the Data Controller:
